IPM Associates, Inc. - Class & Workshop Registration Forms

If interested in attending any of the following classes you will need to fill out a registration form. Select the form you need, complete it on screen, Save and Email it back to us, or print it out and completely fill-in by hand, then scan and email back to us. Our office address is found at the top of each Registration Form, as well as on our Contact Us page. Fees for the classes can be paid at the door on class day by cash, check or credit card. Be reminded that all classes are by registration only and space in some classes is limited, so please register as early as possible.

.PDF files are cross-platform files that can be viewed and printed on any computer that has the free Acrobat Reader software installed on it. Acrobat Reader can be downloaded for free from Adobe.

To download and print these forms, simply click on any .PDF link in this page and it will be downloaded to your computer. It should automatically open and display in your Acrobat Reader plugin. From there you can print and/or save it as desired. If the file is not opened up in Acrobat Reader automatically, right-click on the desired .PDF link and "Save Target as..." the file to a location your computer and then open the .PDF file with Acrobat Reader manually.
